I need an order form created in PHP and integrated into my website. The order form will be very similar to the form here: r-e-v-i-s-i-t-o-r-s.com If you can use some Ajax programming with the form to make it easier to use for my visitors that would be a plus. I am open to expert advice/suggestions.
