Need someone to develop a database with a front end form for data entry into the database.

Can provide sample forms, and detailed field restictions, and could provide the front end form, just need it connected to a back end database (MS Access preffered) so that users can enter data into a form, click save/update and the information will be automatically stored into a database. Around 40 Fields, some text, some check boxes, some numeric, some alpha, etc....

Want to host the Database on and Form on a SharePoint system, but that shouldnt be too much of a problem, as all those entering data in the forms will have appropriate access to the relevant folders on SharePoint.
